If you or a covered dependent is over age 65 and will be enrolling in an HPE-sponsored Medicare HMO, you may need to complete an HMO Medicare assignment form in addition to calling the HPE Benefits Center and enrolling in Medicare. You can request the necessary forms from the HPE Benefits Center, and you’ll need to return them as quickly as possible. It’s a good idea to start this process well in advance of your actual retirement date. Your forms should be submitted to the HPE Benefits Center approximately one month before you want HPE-sponsored Medicare HMO coverage to begin.
